2. KsK SMP Coop online application includes:

• Online Application Form (Submitted)
• 2 pcs. 1x1 I.D. Picture
• Photocopy of 2 identification cards or documents
such as: Philippine Passport / SSS / Voters ID
Driver’s License / PhilHealth ID / TIN Card
Postal ID / Senior Citizen ID / OFW ID

3. You may submit your scanned/photographed copy of requirements via email; info@kskcoop.com. Wait for our confirmation message.

4. Payment procedure:
